ABSTRACT

OBJECTIVE@#To provide prenatal diagnosis, pedigree analysis and genetic counseling for a pregnant woman who had given birth to a child featuring global developmental delay.@*METHODS@#A pregnant woman who underwent prenatal diagnosis at the Affiliated Hospital of Southwest Medical University in August 2021 was selected as the study subject. Peripheral blood samples were collected from the woman, her husband and child, in addition with amniotic fluid sample during mid-pregnancy. Genetic variants were detected by G-banded karyotyping analysis and copy number variation sequencing (CNV-seq). Pathogenicity of the variant was predicted based on the guidelines from the American College of Medical Genetics and Genomics (ACMG). Candidate variant was traced in the pedigree to assess the recurrence risk.@*RESULTS@#The karyotypes of the pregnant woman, her fetus, and affected child were 46,XX,ins(18)(p11.2q21q22), 46,X?,rec(18)dup(18)(q21q22)ins(18)(p11.2q21q22)mat and 46,XY,rec(18)del(18)(q21q22)ins(18)(p11.2q21q22)mat, respectively. Her husband was found to have a normal karyotype. CNV-seq has revealed a 19.73 Mb duplication at 18q21.2-q22.3 in the fetus and a 19.77 Mb deletion at 18q21.2-q22.3 in her child. The duplication and deletion fragments were identical to the insertional fragment in the pregnant woman. Based on the ACMG guidelines, the duplication and deletion fragments were both predicted to be pathogenic.@*CONCLUSION@#The intrachromosomal insertion of 18q21.2-q22.3 carried by the pregnant woman had probably given rise to the 18q21.2-q22.3 duplication and deletion in the two offspring. Above finding has provided a basis for genetic counseling for this pedigree.

ABSTRACT

OBJECTIVE@#To investigate the influence and mechanism of atorvastatin on glycolysis of adriamycin resistant acute promyelocytic leukemia (APL) cell line HL-60/ADM.@*METHODS@#HL-60/ADM cells in logarithmic growth phase were treated with different concentrations of atorvastatin, then the cell proliferation activity was measured by CCK-8 assay, the apoptosis was detected by flow cytometry, the glycolytic activity was checked by glucose consumption test, and the protein expressions of PTEN, p-mTOR, PKM2, HK2, P-gp and MRP1 were detected by Western blot. After transfection of PTEN-siRNA into HL-60/ADM cells, the effects of low expression of PTEN on atorvastatin regulating the behaviors of apoptosis and glycolytic metabolism in HL-60/ADM cells were further detected.@*RESULTS@#CCK-8 results showed that atorvastatin could inhibit the proliferation of HL-60/ADM cells in a concentration-dependent and time-dependent manner (r=0.872, r=0.936), and the proliferation activity was inhibited most significantly when treated with 10 μmol/L atorvastatin for 24 h, which was decreased to (32.3±2.18)%. Flow cytometry results showed that atorvastatin induced the apoptosis of HL-60/ADM cells in a concentration-dependent manner (r=0.796), and the apoptosis was induced most notably when treated with 10 μmol/L atorvastatin for 24 h, which reached to (48.78±2.95)%. The results of glucose consumption test showed that atorvastatin significantly inhibited the glycolytic activity of HL-60/ADM cells in a concentration-dependent and time-dependent manner (r=0.915, r=0.748), and this inhibition was most strikingly when treated with 10 μmol/L atorvastatin for 24 h, reducing the relative glucose consumption to (46.53±1.71)%. Western blot indicated that the expressions of p-mTOR, PKM2, HK2, P-gp and MRP1 protein were decreased in a concentration-dependent manner (r=0.737, r=0.695, r=0.829, r=0.781, r=0.632), while the expression of PTEN protein was increased in a concentration-dependent manner (r=0.531), when treated with different concentrations of atorvastatin for 24 h. After PTEN-siRNA transfected into HL-60/ADM cells, it showed that low expression of PTEN had weakened the promoting effect of atorvastatin on apoptosis and inhibitory effect on glycolysis and multidrug resistance.@*CONCLUSION@#Atorvastatin can inhibit the proliferation, glycolysis, and induce apoptosis of HL-60/ADM cells. It may be related to the mechanism of increasing the expression of PTEN, inhibiting mTOR activation, and decreasing the expressions of PKM2 and HK2, thus reverse drug resistance.

OBJECTIVE@#To evaluate the capacity and efficiency of human umbilical cord mesenchymal stem cells (HUCMSCs) to differentiate into neuron- like cells after induction with B27- supplemented serum- free medium.@*METHODS@#HUCMSCs at passage 4 were cultured for 14 days with serum-containing medium (SCM) (group A), SCM supplemented with 20 ng/mL nerve growth factor (NGF) and 10 ng/mL basic fibroblast growth factor (bFGF) (group B), serum-free medium (SFM) (group C), or SFM supplemented with 20 ng/mL NGF and 10 ng/mL bFGF. The culture medium were changed every 3 days and the growth of the neurospheres was observed using an inverted microscope. The cell markers were analyzed with flow cytometry and the expressions of nestin, neuron- specific enolase (NSE), neurofilament heavy polypeptide (NEFH), and glial fibrillary acidic protein (GFAP) were quantified by quantitative real-time PCR (qRT-PCR) and Western blotting.@*RESULTS@#Before induction, HUCMSCs expressed abundant mesenchymal stem cell surface markers including CD29 (99.5%), CD44 (49.6%) and CD105 (77.7%). Neuron-like cells were observed in the cultures on days 7, 10, and 14, and the cell differentiation was the best in group D, followed by groups C, B and A. In all the 4 groups, the cellular expressions of nestin and GFAP gradually lowered while those of NEFH and NSE increased progressively. The expressions of GFAP, NEFH, nestin and NSE were significantly different between group A and the other 3 groups ( < 0.001 or 0.05).@*CONCLUSIONS@#B27-supplemented SFM effectively induces the differentiation of HUCMSCs into neuron- like cells, and the supplementation with cytokines (NGF and bFGF) strongly promotes the cell differentiation.

ABSTRACT

Abstract Purpose: To investigate the role of vagus nerve activation in the protective effects of hypercapnia in ventilator-induced lung injury (VILI) rats. Methods: Male Sprague-Dawley rats were randomized to either high-tidal volume or low-tidal volume ventilation (control) and monitored for 4h. The high-tidal volume group was further divided into either a vagotomy or sham-operated group and each surgery group was further divided into two subgroups: normocapnia and hypercapnia. Injuries were assessed hourly through hemodynamics, respiratory mechanics and gas exchange. Protein concentration, cell count and cytokines (TNF-α and IL-8) in bronchoalveolar lavage fluid (BALF), lung wet-to-dry weight and pathological changes were examined. Vagus nerve activity was recorded for 1h. Results: Compared to the control group, injurious ventilation resulted in a decrease in PaO2/FiO2 and greater lung static compliance, MPO activity, enhanced BALF cytokines, protein concentration, cell count, and histology injury score. Conversely, hypercapnia significantly improved VILI by decreasing the above injury parameters. However, vagotomy abolished the protective effect of hypercapnia on VILI. In addition, hypercapnia enhanced efferent vagus nerve activity compared to normocapnia. Conclusion: These results indicate that the vagus nerve plays an important role in mediating the anti-inflammatory effect of hypercapnia on VILI.

ABSTRACT

Objective To discuss the application of ultra-thin flap technology in the thoracodorsal artery per forator flap,and appraise the effectiveness in clinical practice.Methods From May,2013 to December,2015,10 patients with soft-tissue defects of the four limbs were treated with ultra-thin thoracodorsal artery perforator flap.The classification of the perforating vessels was done by Kimura method.The flap blood supply was measured and the patients' satisfaction of the appearance of flap was investigated through questionnaires.Results The flap of 8 cases (5 cases of type Ⅰ and 3 cases of type Ⅱ) could be evenly thinned,and the flap of 2 cases (type Ⅲ) could only be thinned at the edge beyond the part with perforator as the center and a radius of 3 centimeters.The flap of 9 cases completely lived.While in 1 case (type Ⅲ),a small part of the edge of the flap was necrosed(2 cm×1 cm),and coalesced well after change dressing.The donor site was sutured directly in 7 cases and covered with skin graft in 1 case.Eight patients were followed-up from 6 to 12 months.The treatment effect was satisfied.And there were no side effects on the functioning of donor sites.Seven patients were highly satisfied with the appearance of the flap (satisfaction rate was 87.5%).Conclusion Ultra-thin flap technology can be adopted to cut thin thoracodorsal artery perforator flap,and can remarkably improve the effect in clinical practice.

ABSTRACT

Objective To explore the association between estimated glomerular filtration rate (eGFR) and metabolic syndrome (MS) . Method The data were from the China Health and Nutrition Survey (CHNS) . The data included the physical measurement and biochemical markers of adults of CHNS in 2009. Finally, 7309 individual observations were included in the analysis. Results According to the criteria of MS, 2034 patients were diagnosed as MS, and the prevalence was 27.83%. There were 1508 (74.14%) patients had moderate reduced eGFR in MS patients, and 3357 individuals (63.64%) were diagnosed as moderate reduced eGFR in non-MS patients. There was significant difference between two groups (X2=72.13, P<0.001) . The incident of moderate reduced eGFR was positively associated with fasting plasma glucose (OR=1.22, 95%CI: 1.06-1.42, P=0.005), triglyceride (OR=1.21, 95%CI: 1.05-1.38, P=0.005) and negatively associated with HDL-C (OR=1.23, 95% CI: 1.06-1.41, P=0.004) . Conclusion The MS and its components might be the influencing factors of developing moderate reduced eGFR, and the screening should be focused on the population with MS.

ABSTRACT

OBJECTIVE:To investigate the effect of prostaglandin E1 combined with atorvastatin on related indicators in elderly patients with hypertensive nephropathy. METHODS:70 elderly patients with hypertensive nephropathy were randomly divided into control group and observation group. Control group was orally given 10 mg Atorvastatin calcium tablet,once a day;observation group was additionally given 10 μg Prostaglandin E1 injection,adding into 250 ml 0.9% Sodium chloride injection by intravenous infusion,once a day. The treatment course for both groups was 2 weeks. All patients were given quality low-protein,low-salt and low cholesterol diet,depressurization,lipid-lowering and other conventional treatment. Heart rate (HR),systolic blood pressure (SBP),diastolic blood pressure(DBP),triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C), low-density lipoprotein cholesterol(LDL-C),serum creatinine(Scr),24h urinary protein(TP/24 h),microalbuminuria(mAlb),uri-nary β2-microglobulin (β2-MG),blood urea nitrogen (BUN) before and after treatment,and incidence of adverse reactions in 2 groups were observed. RESULTS:After treatment,HR,SBP,DBP,TG,TC and LDL-C in 2 groups were significantly lower than before,and difference was statistically significant(P0.05);Scr,TP/24 h,mAlb,β2-MG and BUN were signifi-cantly lower than before,and observation group was lower than control group,the differences were statistically significant(P0.05). CONCLUSIONS:Based on the con-ventional treatment,prostaglandin E1 combined with atorvastatin can effectively improve the HR,blood pressure and blood lipid of elderly patients with hypertensive nephropathy;however,prostaglandin E1 combined with atorvastatin is better than atorvastatin alone in aspect of protecting renal function,with similar safety.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of human umbilical cord mesenchymal stem cells (hUC-MSC) on VP16-induced apoptosis of Nalm-6 cells.</p><p><b>METHODS</b>hUC-MSC were isolated and identified using morphological observation and flow cytometry, then Nalm-6 cells were treated with hUC-MSC with or without VP16. Apoptosis and cell cycle were assayed by FACS. The mRNA levels of apoptosis-related genes BCL-2, BAX and caspase-3 were detected by quantitative RT-PCR, and the protein levels of BCL-2, BAX and caspase-3 were examined by Western blot.</p><p><b>RESULTS</b>FACS showed that hUC-MSC inhibited the proliferation and decreased apoptosis of Nalm-6 cells resulted from VP16. The quantitative RT-PCR showed that hUC-MSC increased the mRNA expression level of BCL-2 and decreased the expression level of BAX and caspase-3 (P < 0.05). Western blot showed that the protein expression level of BCL-2 increased, and expression level of BAX and caspase-3 decreased in Nalm-6 cells after co-culture with hUC-MSC (P < 0.05).</p><p><b>CONCLUSION</b>hUC-MSC may protect Nalm-6 cells from apoptosis induced by VP16 through regulation of BCL-2, BAX and caspase-3.</p>

ABSTRACT

The main objective of this research is to observe protective effects of three phenylallyl compounds(cinnamyl alcohol,cinnamaldehyde and cinnamic acid)from Guizhi decoction against ox-LDL-induced oxidative stress injury on human brain microvascular endothelial cells(HBMEC).In this study,the toxicity and optimal protective concentration of three phenylallyl compounds from Guizhi decoction were determined by MTT assay.The HBMEC were divided into control group(DMSO),model group(ox-LDL),tert-butylhydroquinone (t-BHQ) group,cinnamyl alcohol group, cinnamaldehyde group and cinnamic acid group.The model group were treated with ox-LDL (50 mg•L⁻¹)for 24 h,other groups were separately treated with t-BHQ, cinnamyl alcohol, cinnamaldehyde and cinnamic acid of 20 μmol•L⁻¹, and exposed to ox-LDL (50 mg•L⁻¹) for 24 h at the same time.The survival rate of HBMEC was detected by MTT assay,reactive oxygen species(ROS) production of injured cells were detected using laser scanning confocal microscope (LSCM),the content of SOD, MDA, eNOS and NO in HBMEC was determined by ELISA, and the expressions of Nrf2 mRNA were detected by quantitative Real-time PCR(qRT-PCR).The results shows that oxidative stress injury of HBMEC could be induced by ox-LDL, the three phenylallyl compounds from Guizhi decoction did not affect morphology and viability of normal HBMEC.Compared with model group, the three phenylallyl compounds from Guizhi decoction could improve the above oxidative stress status and up-regulate Nrf2 mRNA expressions in injured HBMEC(P<0.05, P<0.01) .These findings suggested that the three phenylallyl compounds from Guizhi decoction have certain protective effects against ox-LDL-induced oxidative stress injury on HBMEC(cinnamaldehyde> t-BHQ> cinnamic acid>cinnamyl alcohol),the protective mechanism maybe related to regulation of antioxidant enzymes gene expression in HBMEC by Nrf2.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical effect of anastomosis of superficial veins for improving the drainage of perforator propeller flaps.</p><p><b>METHODS</b>From Sept. 2011 to Dec. 2012, 11 cases with soft tissue defects and chronic ulcer wound at extremities were treated with adjacent perforator propeller flaps, which were pedicled by the peroneal artery(5 cases), or the lateral supramalleolar artery(3 cases), or the ulnar artery (2 cases), or the posterior interrosseous artery (1 case). The wound size ranged from from 3.0 cm x 2. 5 cm to 11. 0 cm x 4. 0 cm, and the falps size ranged from 6 cm x 3 cm to 21 em x 5 cm. One superficial vein in all the flaps was anastomosed with superficial vein in the recipient area. The blood supply of the flaps were recorded after operation 1 - 3 months after operation, the fluency of anastomosed vein was detected by color Doppler ultrasound. Flap swelling evaluations were performed in early 3 months and later 3 - 6 months, and the results were classified into 4 grading degrees. 6 months later, Questionnaire of the flap aesthetic satisfactory was performed for seven patients during follow-up period.</p><p><b>RESULTS</b>9 flaps survived completely, two flaps had partial marginal skin necrosis in the distal end, which were both managed with surgical debridement, and both wounds healed in two months. 9 cases were followed up for more than 12 - 19 months. The early rsults of flap swelling evaluations were: I degree 0 case, II degree 8 cases, III degree 3 cases, IV degree 0 case, and the later results were: I degree 7 cases, II degree 4 cases, III degree 0 case, IV degree 0 case. The flaps had ideal appearance, good contour, and high aesthetic satisfactory (100%). The mean flap survival area rate of veins anastomosed was (98. 6 ± 9. 7) %.</p><p><b>CONCLUSIONS</b>Perforator propeller flaps with anastomosis of superficial veins can improve the flap venous drainage, avoid transient venous venous congestion, so as to increase the flap survival. It is an effective way for improving the vein drainage.</p>

ABSTRACT

Objective To investigate the methods and results of lateral leg propeller pedicled flaps based on multi-perforators for coverage of soft-tissue defects on the foot and ankle.Methods From May,2012 to June,2013,8 patients with soft-tissue defects were treated,including 5 cases by trauma,2 cases by chronic unlcers,1 case by infection on the foot and ankle with exposed osseous and tendinous.Lateral leg propeller pedicled flaps based on multi-perforators were elevated and rotated with the angle from 150° to 180° for coverage of soft-tissue de-fects.Flap size ranged from 15.0 cm × 7.0 cm to 23.0 cm × 9.0 cm.Skin graft was applied to cover the donor sites.After the operation,the blood supplies of flaps were observed severely over 10 days.Questionnaire of the flap aesthetic satisfactory and AOFAS evaluation were performed in 7 patients during fellowed-up periods.Results Seven flaps were all survived well,1 flap had partial marginal skin necrosis on the distal,which was managed with surgical debridement,and wound healed in 1 month.Seven patients had a mean of 10.7 months' fellowed-up periods.The flaps had like-like appearance,good contour,and high aesthetic satisfactor (100%).The mean AOFAS score was 90.5.Conclusion Lateral leg propeller pedicled flaps based on multi-perforators can supply the blood of the larger volume of tissue,which can be safely sustained to repair distal and larger defects on the foot and ankle.

ABSTRACT

Objective To investigate the effect of different temperature moxibustion on serum IL-1β, IL-2 and TNF-αcontents in acute adjuvant arthritis rats and provide a basis for the mechanism of local anti-inflammatory and immune action of moxibustion. Methods A rat model of adjuvant arthritis (AA) was made by Freund’s complete adjuvant (FCA) modeling method. Of 32 SD rats, 8 were randomly selected as a normal group and the other 24 for model making. After successful model making, the rats were randomly allocated to model, treatment 1 and treatment 2 groups, 8 rats each. Treatment 1 and 2 groups received moxibustion. Local temperature at moxibustion point was controlled at (38±1)℃in treatment 1 group and at (45±1)℃in treatment 2 group. Rat serum IL-1β, IL-2 and TNF-αcontents were measured by enzyme-linked immunosorbent assay (ELISA). Results There were statistically significant differences in serum IL-1β, IL-2 and TNF-αcontents between the model, treatment 1 or treatment 2 group and the normal group (P<0.01). There was a statistically significant difference in serum IL-2 content between treatment 1 and the model groups (P<0.01). There were statistically significant differences in serum IL-1β, IL-2 and TNF-αcontents between treatment 2 group and the model or treatment 1 group (P<0.01). Conclusions Moxibustion has a reducing effect on serum IL-1βand TNF-αcontents and a raising effect on IL-2 content. 45℃moxibustion temperature can improve the effect of moxi bustion. The anti-inflammatory action of moxibustion may be through the mechanism of reducing serum IL-1βand TNF-αcontents and raising IL-2 content, which relieve body inflammatory reaction. The action of moxibustion needs proper moxibustion temperature.

ABSTRACT

Objective To investigate the types and frequency of gene mutations of β‐thalassemia in south of Sichuan area ,so as to provide basis for β‐thalassemia prevention plan and to help drop β‐thalassemia incidence effectively .Methods Polymerase chain reaction(PCR)and reverse dot blot(RDB)techniques were employed to perform diagnostic analysis for β‐thalassemia genes in sus‐pected thalassemia patient in south of Sichuan region.Results The detection rate of β‐thalassemia was 60 .1% among 319 cases ;9 genes mutation types was found among 17β‐thalassemia genes mutation types .Three types of gene mutations had highest frequency of occurrence ,followed by CD17 41 .3% ,CD41‐42 27 .39% ,IVS‐Ⅱ‐654 24 .35% .Conclusion The gene mutation rate of β‐thalasse‐mia are higher in south of Sichuan region .In order to prevent the birth of children with intermedial and major thalassemia ,the sig‐nificance is important to strengthen hematology screenings and genetic diagnosis for β‐thalassemia.

Objective To investigate the methods and clinical effects of flow-through posterior interrosseous artery flaps for reconstructing digital wounds combined with artery injury.Methods From December 2011 to September 2013,11 patients with traumatic digital defect combined with artery injury were repaired with flow-through posterior interrosseous artery flaps.Flap aesthetic satisfaction was assessed using questionnaires; flaps were tested for pain and touch sensation,temperature perception,and two-point discrimination; finger activity was detect using the total active movement (TAM) ratio of the injured and uninjured side.Results All flaps and injured fingers survived.Wounds healed by the first intension.Nine patients were followed up for 12-24 months.Two-point discrimination of the flaps recovered at postoperative 6 months and averaged (9.7 ± 2.1) mm at postoperative 12 months,with S3 +grade in sensation evaluation.TAM ratio of the injured and uninjured side was 0.85 ±0.17.All patients expressed the satisfaction to flap appearance.Conclusions Flow-through posterior interrosseous artery flaps are safe and effective to cover digital wounds and restore blood circulation.The technique improves the clinical application of perforator flaps.

ABSTRACT

To study the medication regularity of Huazhuo Jiedu Formula in treating the chronic atrophic gastritis (CAG), and to explore the stable relationship of correspondence between Huazhuo Jiedu Formula and the pattern of internal accumulation of zhuodu. Statistical analysis was conducted among 200 cases of CAG, whose case histories included their gender, age, electronic gastroscope, pathological results, clinical symptoms, and prescriptions of Chinese materia medica. On the basis of "golden section method", the association rule between drugs and syndromes was investigated using frequency statistic method and association rule analysis. Through data processing, 153 kinds of Chinese herbal decoction were obtained. According to the principle of "golden section method", 15 kinds of Chinese herbal decoction, whose frequency of use was over 61.8% (including 61.8%), were used as the therapeutical core drugs of pattern of internal accumulation of zhuodu in CAG. Ten kinds of clinical symptoms, which had the high ocurrence rate, were the main symptoms of the pattern of internal accumulation of zhuodu in CAG. According to association rules analysis, it was found that there was a strong correlation between drugs and pulses, which explained the high degree of correspondence of prescription and syndromes between the pattern of internal accumulation of zhuodu in CAG and Huazhuo Jiedu Decoction. It can be seen that there exists a relationship of correspondence between Huazhuo Jiedu Formula and the pattern of internal accumulation of zhuodu in CAG. And it shows that the key pathogenesis of CAG is internal accumulation of zhuodu, and the corresponding treatment is Huazhuo Jiedu Method.

ABSTRACT

In the book of Chinese Acupuncture and Moxibustion by CHENG Dan-an, direct moxibustion with moxa cone was regarded as a classic style of moxibustion because of its innate character of heat induced thermal stimulus and burn toxin. CHENG Dan-an made the quantity of moxibustion more controllable through the selection of moxa wool, the size and number of moxa cones, and the tightness of moxa cones, etc. CHENG Dan-an 's clinical experience is summarized through treating stubborn and infectious diseases with direct moxibustion with moxa cone. The results showed that only with understanding the characteristics of moxibustion and key pathological mechanism of suitable disease could direct moxibustion with moxa cone and the appropriate methods be performed correctly. CHENG Dan-an confirmed the clinical effect of treating stubborn and intectious diseases with direct contact moxibustion with moxa cone, and optimized the specific operation scheme of direct moxibustion with moxa cone. It tell us that we should deeply study on moxibustion, to make it wide spread in the hereafter, bring benefit to mankind.

ABSTRACT

This study was purposed to establish a new quick and simple diagnostic method with high sensitivity and good specificity for idiopathic thrombocytopenic purpura (ITP) and to evaluate its significance. 240 platelet lysates (from patients with ITP, leukemia, MDS, and healthy adults, each of 60 cases) were randomly assigned to training set (120 cases) or validation set (120 cases), all of them were detected by surface enhanced laser desorption ionization time of flight mass spectrometry (SELDI-TOF-MS), in order to identify the differentially expressed protein, the diagnostic model was established by means of artificial neural network (ANN), and was validated by blind test with SPSS 17.0. The results showed that 5 marked proteins significantly differentially expressed (P < 0.01), m/z of highly expressed proteins were 2234.30, 3476.36, and 7526.29, m/z of low expressed proteins were 4990.02 and 5152.39, respectively. The sensitivity and specificity of diagnostic model were 80.6% and 77.3% respectively. The area under the ROC curve consisting of the output value of artificial neura1 network was 0.837. Efficacy of the model was validated by means of blinded test. It is concluded that the ANN model is useful for clinical diagnosis of ITP on the basis of platelet protein fingerprint spectrum.

ABSTRACT

<p><b>OBJECTIVE</b>Acute pulmonary thromboembolism (PTE) is a serious high mortality pulmonary vascular disease whose effective treatment decreases morbidity and mortality. To determine if low-molecular-weight-heparin (LMWH) is clinically as efficient and safe as unfractionated heparin (UH) in patients with diagnosis of acute non-massive PTE, our study compares the efficacy, adverse effects and costs of LMWH and UH.</p><p><b>METHODS</b>One hundred and fourteen patients with non-massive acute PTE were randomly divided into LMWH (nadroparin calcium) and UH groups. Oxygenation index, D-dimer, fibrinogen (FG), lung ventilation/perfusion (V/Q) scan and computed tomography pulmonary angiography (CTPA) were observed before anticoagulation and on day 14 after anticoagulation.</p><p><b>RESULTS</b>In both groups, the ABG (arterial blood gas) analysis showed PaO(2) and PaCO(2) were elevated, P(A-a)O(2) was decreased and oxygenation index (PaO(2)/FIO(2)) was elevated, D-dimer and fibrinogen were decreased, lung V/Q and CTPA showed embolized segments reduced (P<0.05). Hemorrhage and thrombocytopenia occurred in 3.5% of the LMWH group. Hemorrhage occurred in 5.3% and thrombocytopenia occurred in 7.0% of the UH group. The average cost in the LMWH group was RMB 1218.60 Yuan and RMB 1541.40 Yuan in the UH group.</p><p><b>CONCLUSION</b>LMWH and UH are equally effective for treatment of non-massive acute PTE, but LMWH may have a lower prevalence of complications and is less expensive.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the epidemiology of Acinetobacter baumannii infection in patients with ventilator-associated pneumonia (VAP).</p><p><b>METHODS</b>Prospective clinical study was carried out with 176 episodes of VAP with etiologic diagnosis being followed in two groups.</p><p><b>RESULTS</b>Twenty-six episodes were caused by Acinetobacter baumannii and one hundred-fifty episodes were caused by "other" organisms. Using logistic regression analysis, the risk of VAP due to Acinetobacter baumannii was found to be high in patients with head trauma [odds ratio (OR) = 4.20, 95% confidence interva (CI): 2.72 to 6.48], surgery (OR = 2.88, 95% CI: 1.78 to 4.66), acute respiratory dispnea syndrome (OR = 2.81, 95% CI: 1.19 to 6.64), and large-volume pulmonary aspiration (OR = 6.71, 95% CI: 3.91 to 11.50).</p><p><b>CONCLUSIONS</b>Acinetobacter baumannii pulmonary infection in incubated patients had an epidemiological pattern that different from "other" organisms. Patients with high risk identified in our study might mark the existence of cross-infection during airway manipulation.</p>
